What is Collaborative Care? |

Our Collaborative Care program provides 90-day, wrap-around support plans that can be extended as often as needed, so no one slips through the cracks. Through supportive methods that address the effects of past trauma and honor everyone's cultural background, individuals receive vital connections and referrals to mental health services, substance use treatment, housing, or other essential resources. By uniting multiple providers and services under one plan, Collaborative Care reduces confusion and wait times. This model ensures that each person is "wrapped" in continuous care, preventing them from getting lost in a fragmented system. |

| Overseeing this life-changing program is our Director of Collaborative Care, Chavelyn Santana. Chavelyn introduced the concept of 90-day support plans when she recognized a need in the community for more structured, longer-term support. Chavelyn — a native Spanish speaker —brings her extensive experience working with multigenerational and marginalized populations. Her compassion is felt in every client's journey. By ensuring each support plan is trauma-informed and culturally sensitive, she helps clients feel truly heard, seen, and empowered. |
